Kindergarten registration March 12-16
Ada Schools kindergarten registration will be held from 8:45 until 3 p.m.during the week of March 12-16. Also, registration will be offered on Tuesday from 5-7 p.m.
Here are the details:
Your child does not have to attend registration. During registration you will be able to set up a time for your child’s kindergarten screening to be held May 7 or May 8. Your child will need to come with you for screening in May.
At the time of registration you must bring the following:
Child’s birth certificate
Child’s social security card
Child’s immunization record
Custody papers, if applicable
Proof of residency (driver’s license, utility bill, rental agreement or other item listing your current address)
Your child’s registration will not be completed without these documents. Registration will take 30-45 minutes.
Immunization requirement for kindergarten entrance are:
*5 doses of DTaP/DTP/DT (Diphtheria, Tetanus, Pertussis)
*4 doses of Polio
*2 doses of MMR (Measles, Mumps, Rubella)
*3 doses of Hep B (Hepatitis B)
*2 doses of Varicella (Chickenpox)
*HIB is NOT required
If immunization requirements are incomplete at registration, the school will ask you to get what is needed and provide the new dates to the school nurse prior to the first day of school.
If immunizations are still not completed by the first day of school year, or if you have not provided the documents listed above, your child may not be allowed to attend school until all missing documents are on file with the school.
