Ada Rite Aid free vaccination clinic March 7

Ada Rite Aid Pharmacy will be holding a free covid-19 vaccination clinic at Ada First United Methodist Church on Sunday, March 7.
Vaccinations will begin at noon in the church Fellowship Hall on the first floor.
This clinic will be for the first dose of the Pfizer vaccine. The second dose is scheduled Sunday, March 28, at noon,

You must complete the screening questionnaire prior to your arrival and qualify for the vaccine by meeting one or more of the following criteria…
• Age 60 or above
• Or have a long-term medical condition such as: heart disease or cardiac issues, Type I or II diabetes, ALS.
• Or Pregnant Women, Law Enforcement or Corrections Officers, Child Care Providers (licensed programs), Funeral Service employees
Please bring the following items with you on March 7th (there is no cost for this vaccine):
• Completed Screening Questionnaire
• Driver’s License - or birth certificate if no license
• Current Medicare A/B Red, White, & Blue Card
• A Health or Prescription Insurance Card (you can still receive the vaccine even if you do not have health or prescription insurance)
Attached is
• a COVID-19 Vaccination Guide with information about the vaccine and what to expect,
• the Screening Questionnaire which you must complete and bring with you.
