The Ada Exempted Village School District kindergarten registration for the 2013-2014 school year will be held on Thursday, March 14 and Friday, March 15 at Ada Elementary. 

Times will be from 8:30 a.m. to 3:00 p.m. on each day. Your child will not have to attend with you for registration.

At the time of registration you will be able to set up a time for your child’s kindergarten screening to be held on May 7 and 8.  

At the time of registration you must bring your child’s birth certificate, social security card, immunization records, custody papers, if necessary, and proof of residency.  The school will not allow registration without these documents.  Registration will take approximately half an hour.

Immunizations required for Kindergarten entrance are as follows: 

• 5 doses of DTaP/DTP/DT (Diptheria, Tetanus, Pertussis)
• 4 doses of Polio
• 2 doses of MMR (Measles, Mumps, Rubella)
• HIB is not mandatory
• 3 doses of Hep B (Hepatitis B)
• 2 doses of Varicella (Chickenpox)

If immunization requirements are incomplete at registration, the school will ask you to get what is needed and provide the new dates to the school nurse before the first day of school 2013.
